2012-2013 Athletic Participation and Physical Forms: Must be completed prior to trying out for a competitive sports team. 2 page form. Page one to be completed by parent/guardian. Page two to be completed by the child’s healthcare provider.
Driving Record Consent Form; Required by Archdiocese of Baltimore if you will be transporting any children other than your own to or from athletics events.
